This one-hour foundational course is for interprofessional care teams, with a focus on primary care. This course will help you identify, engage, and support caregivers. It covers essential communication, coordination, and systems-wide approaches to improve patients’ health and well-being through engaging caregivers. Free CE/CME.

Identifying the Caregiving Team (15 min)  

This course provides practical ways to identify a patient’s caregiving team and tools for documenting caregiving teams. 

Engaging Caregivers (15 min) 

This course helps learners implement effective communication skills with caregivers and care recipients, uncover any unmet needs of the caregiver, and tailor appropriate interventions. 

Building and Supporting Caregiving Teams (15 min) 

This course provides effective strategies to support caregivers through education and care coordination, ultimately improving the health and well-being of the caregiver and care recipient.
