All CAP-CT courses cover essential communication, coordination, and system-wide approaches to improve patients’ health and well-being through engaging caregivers. Free CE/CME provided.

Foundational Skills for Engaging Caregivers in Health Care (1.0 AMA/ANCC/AAPA/ASWB)

This foundational course will help you identify, engage, and support caregivers. The course is for interprofessional care teams, with a focus on primary care.

Objectives:

  1. Describe the benefits of caregiver engagement in primary care. 

  2. Describe when and how to identify members of a patient’s caregiving team. 

  3. Demonstrate how to include caregivers as an integral part of your health care team. 

  4. Review strategies that promote caregivers' well-being and their ability to support your patients.

Identifying the Caregiver Team (0.25 AMA/ANCC/AAPA)

Engaging Caregivers (0.25 AMA/ANCC/AAPA)

This course helps learners implement effective communication skills with caregivers and care recipients, uncover any unmet needs of the caregiver, and tailor appropriate interventions. This course can be taken on its own or as a supplement to the foundational course. 

Objectives:

  1. Discuss the importance and benefits of engaging caregivers.
  2. Implement effective communication skills with caregivers and care recipients.
  3. Identify if caregivers have unmet needs and then tailor appropriate interventions. 
Supporting Caregivers (0.25 AMA/ANCC/AAPA)

This course provides effective strategies to support caregivers through education and care coordination, ultimately improving the health and well-being of the caregiver and care recipient. This course can be taken on its own or as a supplement to the foundational course. 

Objectives:

  1. Recognize the important role of caregivers in providing patient care.
  2. Identify ways to support caregivers through education and care coordination.
  3. Identify how to leverage community resources to support caregivers. 
Engaging Kin and Grandfamily Caregivers (0.5 AMA/ANCC/AAPA)

This course describes how to engage kin and grandfamily caregivers in primary care, improving patient care and caregiver well-being.

Objectives:

  1. Define kin and grandfamily caregivers.
  2. Describe the benefits of identifying and engaging kin and grandfamily caregivers in the primary care of a minor.
  3. Demonstrate how to include kin and grandfamily caregivers as an integral part of your health care team.
  4. Identify a caregiver's unmet needs and support them with resources to improve their well-being.
