Overview

Identifying and engaging caregivers improves health outcomes and quality of life for both the person receiving care and their caregivers!

During this 12-month learning collaborative, participating health care delivery sites will pilot a care process of their choosing to help teams identify, engage, and support family caregivers. Participants will benefit from peer learning while advancing their own goals for improving caregiver support within their organization. Participants will also be supported by CAP-CT training and resources.

Outcomes from the collaborative will inform an implementation toolkit that will be shared widely to facilitate greater adoption of care processes that help health care teams identify, engage, and support family caregivers.

Eligibility

This collaborative is open to health care delivery sites in adult inpatient settings. Individuals from the participating sites should be in a leadership role with the authority to make operational or process improvements, e.g., for a department, division, unit, etc.

There will be up to five sites selected for this learning collaborative. We aim for diversity in geographic location, organization size, and patient population served.

Participation Stipend

Participating sites will be offered a stipend of $8,000 for the time spent engaged in the learning collaborative, as outlined in the participant expectations below. The stipend will be paid in two installments - one at the start of the program and one at the end of the program.

Participant Expectations

To ensure value and impact, each participating site will be expected to:

1. Pilot a process that helps identify, engage, and support family caregivers such as:

  • Implementing a new caregiver documentation workflow in the EHR.
  • Using a screening tool to gauge caregiver strain (e.g., Zarit burden interview).
  • Referring older adults and their caregivers to free legal and financial care planning (e.g., by utilizing Clarity Pro).

2. Participate in learning sessions

During quarterly learning sessions, participants will engage in peer learning to help advance their care process. The final session will be in-person to discuss results and share plans for sustainability. CAP-CT will cover travel costs for this in-person meeting for up to two individuals per site.

3. Complete project-related requirements and support evaluation activities

  • Assess and report the impact of your care process at four points throughout the program using the PDSA (Plan-Do-Study-Act) framework (approx. 30 min each).
  • Designate key staff to attend at least one CAP-CT training (approx. 60 min).
  • Designate at least two staff members to participate in an interview or focus group as part of the evaluation of the CAP-CT program (approx. 60 min).

Frequently Asked Questions (FAQ)

One requirement is for key staff to attend at least one CAP-CT training. What training and what do you mean by key staff?

Participants are invited to take any CAP-CT training, and we recommend the training specifically for the inpatient setting that will be released in early 2026. Key staff may include those who interact with patients and their caregivers; any staff who are well-positioned to integrate learnings into your organization's workflow. What is the acceptance timeline?

Applications close on November 7, 2025. All applicants will be notified in early December 2025.

Can multiple departments or units within the same organization apply?

Yes, and each application will be reviewed independently.

What if we can't attend every learning session?

Full attendance is required, and we encourage the same person(s) to attend all sessions to support community building and continuity. However, a peer can attend in your place if a conflict arises.

How will stipends be disbursed?

The stipend will be paid in two installments: one at the start of the program and one at the end, contingent upon completion of program expectations.

My organization has a process improvement framework in place - can I use that during this collaborative?

All participants will be required to use the PDSA (Plan-Do-Study-Act) template that we provide. The PDSA template is intentionally simple and standardized so that everyone is working from the same model and progress can be easily shared, assessed, and collated.

How much time will participation require?

We estimate up to 2-3 hours per quarter for learning session participation and evaluation activities, in addition to the time needed to test and implement your chosen care process.

What happens after the collaborative ends?

Participants' lessons learned will be included in an implementation toolkit. We plan to disseminate the implementation toolkit at various venues and conferences across the country. Participants will be encouraged to collaborate on abstract submissions and presentations. We also hope participants will sustain and scale successful caregiver support processes.
