Identifying, Engaging, and Including Caregivers

Identifying and engaging caregivers improves health outcomes and quality of life for both the person receiving care and their caregivers!

During CAP-CT’s learning collaborative, participating health care delivery sites will pilot a care process of their choosing to help teams identify, engage, and support family caregivers. Participants will benefit from peer learning while advancing their own goals for improving caregiver support within their organization. Participants will also be supported by CAP-CT training and resources. The collaborative will take place February 2026 – February 2027. Outcomes from the collaborative will inform an implementation toolkit that will be shared widely to facilitate greater adoption of care processes that help health care teams identify, engage, and support family caregivers.

This learning collaborative is funded by the Administration for Community Living and Archstone Foundation.

Sample Pilot Care Processes
  • Using more in-depth admission questions to help with care at home
  • Implementing an individualized caregiver care plan addressing physical and emotional health
  • Administering stress assessments to identify who is at risk and need connection to services
  • Integrating a caregiver identification tool into the EHR
